Student Application Form

This form should only be used by enrolled student applicants who have no prior psychotherapy qualifications at Diploma level or higher.
 

Membership Renewal Form

Use this form to indicate your intention to either resign or renew your AAHTC membership and to report your professional development.
 

Membership Upgrade Form

Use this form to upgrade your membership category. Phoenix students whose classes finish in December will normally be upgraded automatically from Student to Qualified as part of the membership renewal process.
